Ceano Adaptive Intervention

Ceano Health utilizes a multifaceted approach to intervention influenced by the pioneers of our field. Our model was created by Ceano’s Founder, Danny Smith, through his decades of experience working with individuals, families, and organizations in behavioral health and having had the privilege of being mentored by leading interventionists in the field. The Ceano Adaptive Intervention Model draws from the existing models of intervention and integrates them to form a hybrid, flexible methodology that can be tailored specifically to meet the needs of each presenting issue. Ceano Adaptive Intervention Model is a custom intervention technique that is designed to accommodate the clinical and medical needs of the individual and family or concerned parties, as well as incorporate elements such as risk assessment and risk mitigation, to create a higher level of predictability and response preparation.

With our comprehensive network of providers and resources, as well as our customizable intervention model, our team has extensive experience in supporting intervention needs for:

  • Addiction and substance use disorders

  • Process addictions

  • Primary mental health disorders

  • Trauma resolution

  • Family relationships

  • Safety

Care Management

Ceano’s Care Management programs offer support at varying stages of recovery and for differing levels of need. Our three lines of service within the Care Management Program are designed to provide options for what is best for you, your loved one, or your client, based on our team’s thorough assessment and discussions with you. We serve clients all around the world with our individualized care management curriculum.

In addition to our traditional Care Management services, we offer experiential and intensive workshops as well as safety options that can be added to any agreement.

  • Our most comprehensive Care Management Agreement that includes in-person facilitation and implementation of the Life Balancing Assessment and Project as well as all continuing care discharge plans that client receives from current or previous treatment providers; twice weekly in-person mentoring; daily telephone support; video conferencing; collaboration with client’s treatment and/or outside care team; referrals, appointments and follow-ups with any additional providers such as psychiatric, medical, or clinical professionals as needed; accountability for attendance of all medical, clinical, self-care, and recovery appointments including recovery meetings if included in client’s care plan; and more. Our telehealth and virtual components utilize secure, encrypted HIPAA compliant communication systems.

  • Our Tier II Care Management program provides a virtual option through video conferencing and telehealth. Tier II offers similar services to Tier I such as facilitation and implementation of the Life Balancing Assessment and Project, support in implementing continuing care or discharge plans from current or previous treatment providers, weekly video conference mentoring sessions, weekday telephone support, collaboration with client’s treatment and/or outside care team; referrals, appointments and follow-ups with any additional providers such as psychiatric, medical, or clinical professionals as needed; accountability for attendance of all medical, clinical, self-care, and recovery appointments including recovery meetings if included in client’s care plan; and more. Our telehealth and virtual components utilize secure, encrypted HIPAA compliant communication systems.

  • Tier III Care Management is dedicated to supporting individuals and families while a client is residing in care, such as a hospital setting or residential treatment facility. Our In-Treatment Support (ITS) services maintain a bridge between a client’s in-treatment care team and the family or outside recovery support network. Our team collaborates with the hospital or residential facility to provide an additional layer of support for facility staff, clients, and family and/or a client’s support network. These services include as-needed phone support, weekly check-ins with client and treatment team to receive updates regarding progress and treatment planning, medication changes and compliance, recommended length of stay in treatment, barriers to progress, and discharge and continuing care planning. Additionally, our team will provide in-treatment intervention or consulting services if needed to support clients in remaining in care when there is risk of client leaving against clinical or medical advice. As research has shown that longer retention in treatment, as well as adherence to care plans both within treatment as well as beyond discharge leads to vastly longer term and more positive outcomes, our ITS services focus on supporting clients, facilities, families, and organizations in creating and maintaining a comprehensive treatment experience throughout the duration of care. This tier also streamlines a client’s process of stepping into Tier I or Tier II Care Management upon discharge, maintaining the trust and relationship with their Care Manager.
